At San Bernardino City Unified School District (SBCUSD), our School-Based Health Centers (SBHCs) are dedicated to supporting the holistic well-being of our students. Our case management services play a pivotal role in this mission by offering personalized support to students and their families, ensuring they have access to the necessary resources and services for academic and personal success.

What Are Case Management Services?

Case Management Services involve a collaborative and student-centered approach to identify and address the unique needs of each student. Our team of skilled professionals works closely with students, families, school staff, and community partners to develop and implement comprehensive support plans.

Services Include:

  • Needs Assessment: Evaluating the student's academic, social, emotional, and health needs to create a tailored support plan.

  • Resource Coordination: Connecting families with internal and external resources, such as counseling, healthcare, academic tutoring, and community programs.

  • Advocacy: Assisting families in navigating educational and community systems to ensure students receive appropriate services and accommodations.

  • Monitoring and Follow-Up: Regularly reviewing and adjusting support plans to reflect the student's progress and changing needs.

In partnership with our medical provider, we offer integrated health services that address both physical and mental health concerns. This collaboration ensures that students, families, and community members have seamless access to medical care, health education, and preventive services, all within the school environment.
