San Bernardino City Unified School District emphasizes the importance of meeting immunization requirements before students begin school to protect public health and prevent the spread of communicable diseases. The requirements align with guidelines established by the California Department of Health Services and the Centers for Disease Control and Prevention (CDC).

Required Immunizations

The following immunizations are required at the time of enrollment:

  • Polio, Varicella, Diphtheria, Tetanus, and Pertussis (DTAP) - 5 doses
  • Polio - 4 doses
  • Hepatitis B - 3 doses
  • Measles, Mumps, Rubella (MMR) - 2 doses
  • Varicella (Chickenpox) - 2 doses

Remember to always keep your child’s immunization record and to bring it to the school at the time of registration.

Exemptions

Parents or guardians seeking a medical exemption from required immunizations for their child in the San Bernardino City Unified School District must submit a written statement from a licensed physician (M.D. or D.O.) in California. This statement must include:

  • Justification for Exemption: A description of the child’s medical condition or circumstances that contraindicate the immunization(s).
  • Specific Vaccines: Identification of the vaccines being exempted.
  • Exemption Type: Clarification on whether the exemption is temporary or permanent.
  • Expiration Date: If the exemption is temporary, an expiration date (not exceeding 12 months).

Note: Exemptions are limited to a maximum duration of 12 months.
