Required Health Forms for Students and Families

To receive appropriate care, certain forms must be completed and submitted to your child’s school. These forms ensure that our staff can provide the best care for your child in line with district guidelines, state laws, and individual health needs. Please download and complete the appropriate forms below as needed.

If your student has a medical need, we’re here to help. You can:

  • Speak with a nurse or health aide: Contact your child’s school to talk with a nurse.
  • Reach out to our team: Call the Student Wellness and Support Services department at (909) 473-2080.

Note: All forms must be completed and submitted to the appropriate school health office. Please make sure to keep your child's health information updated annually.
