Immunizations

Overview
For the protection of all students attending schools within the San Bernardino City Unified School District, it is important that all immunization requirements as listed in the "Guide to Immunizations Required for School Entry Grades K-12" are met before a child begins school. These guidelines have been established by the California Department of Health Services and Centers for Disease Control and Prevention.

Medical Exemption
New Immunization Laws

Medical Exemption

A parent or guardian must submit a written statement from a licensed physician (M.D. or D.O.) which states:

That the physical condition or medical circumstances of the child are such that the required immunization(s) is not indicated.
Which vaccines are being exempted.
Whether the medical exemption is permanent or temporary.
The expiration date, if the exemption is temporary.

New Law Effective July 1, 2019

Medical exemptions for new admissions may be signed only by a California licensed MD/DO.
Each temporary medical exemption may be issued for no more than 12 months.
