The new and improved Safety Improvement Report Form allows for real-time reporting and tracking of safety concerns. The QR code/link below may be used to report a condition or activity within the San Bernardino City Unified School District (SBCUSD) that you believe is unsafe or hazardous. This form is not for reporting an accident, incident, or near-miss incident. All information is kept confidential. Once completed, this form is sent electronically to the Safety Office (909-381-1192)
