Immunization Requirements

Overview
For the protection of all students attending schools within the San Bernardino City Unified School District, it is important that all immunization requirements as listed in the "Guide to Immunizations Required for School Entry Grades K-12" are met before a child begins school.  These guidelines have been established by the California Department of Health Services and Centers for Disease Control and Prevention.   For more information regarding these and other recommended immunizations, contact Health Services at (909) 880-6839 or visit the websites for the California Department of Health Services  http://www.cdph.ca.gov/ or Centers for Disease Control and Prevention  http://www.cdc.gov/ .  Remember  immunizations prevent the spread of communicable diseases.

Kindergarten-12th grades

Prior to starting kindergarten or entering school up to grade 12, all students must receive the following shots:
  • Polio (OPV and/or IVP)
    • 3 doses  (if one given on or after 4th birthday for K or 2nd birthday for ages 7-17)                                                                                                                          
    • 4 doses maximum (given at any age)
  • DTP (DTap / DT / Td) - Diphtheria / Tetanus / Pertussis
    • 4 doses  (if one given on or after 4th birthday for K or 2nd birthday for ages 7-17)                                                                                                                          
    • 5 doses maximum (given at any age)
  • MMR (Measles, Mumps, Rubella)
    • 2 doses both on or after 1st birthday K and 7th grades
    • 1 dose on or after 1st birthday (grades 1-6 and 8-12)
  • Hepatitis B
    • 3 doses at any age
  • Varicella (Chicken Pox)
    • 1 dose (for kindergarten and children under 13 years)
    • 2 doses (ages 13-17 for transfer students admitted to California school before July 1, 2001)
    • Or health care provider documentation of varicella disease or immunity 
  • Tdap Booster-7th Grade 1 dose on or after 7th birthday (also grades 8-12 transferring from outside California)
