   To obtain a Work Permit, a student must:


 1.  Be enrolled in school for the minimum school day established by law. (EC 48200 Compulsory Education Law/Children between ages 6 and 18 years)

2.  Attends school regularly; and "shall not be out of school...for a period longer than 10 consecutive days while public schools are in session."  (EC 49100 Mandatory Attendance)

3.  Maintain a G.P.A. of 2.0 with no more than one "F" (Currently established District practice)


4.  Not suspended or expelled (Currently established District practice)


5. Obtain a
Notification of Intent to Employ a Minor form from the high school College and Career ReadinessCenter (EC 49162 Notification of Intent to Employ)


Note:
The Notification of Intent to Employ a Minor shall contain the following information: 

·          The name, address, phone number, and social security number (SSN) of the minor

·         The name, address, phone number, and supervisor at the minor's place of employment

·         The kind of work the minor will perform

·          The maximum number of hours per day and per week the student will be expected to work for the employer 

·         The signature of the parent or guardian, or the minor, and of the employer (EC 49163 Content of Notification)


6. Submit the completed Notification of Intent to Employ a Minor form to the high school Career Center or College and Career Readiness/Linked Learning Office, 4030 Georgia Blvd., San Bernardino, 92407

Note:
Students must bring original documents that will verify their (SSN) and birth date.  A Work Permit to Employ will be issued upon verification of requested information and shall contain the following information:  

·          The name, age, birth date, address, and phone number of the minor 

·         The place and hours of compulsory school attendance  

·         The maximum number of hours per day and per week the student may work while school is in session

·         The minor's (SSN)

·         The signature of the minor and the issuing authority

·         The date on which the permit expires (EC 49115 Contents)


Work Permits are issued to students, who reside within the school district boundaries in the College/Career Center at each high school and the College and Career Readiness/Linked Learning Department at Career Readiness/Linked Learning Office, 4030 Georgia Blvd., San Bernardino, 92407
, by personnel authorized in writing by the superintendent. (EC 49110 Issuance; jurisdiction)
